Keep vital medical information about you and your family at your fingertips with "My Medical Records." This easy-to-use application for PalmOS handhelds lets you easily manage a wide variety of medical information including patients, doctors, immunizations, prescriptions, allergies, conditions, medical events, medical exams, insurance, clinics and more. Lots of ease-of-use features are built-in, such as smart "auto-fill" to make data entry a breeze, and a robust database engine lets you store a virtually unlimited amount of information. If you are in the "Sandwich Generation" (you take care of aging parents as well as your children), this is a must-have for your PalmOS handheld!

 

The Demo version allows data entry for 30 days after installation. Afterward, you can still view all previously entered data, but you cannot create any new records without registering the software.

System Requirements

Palm OS 3.1 or higher.

 

Installation and Usage

Installation

If you are upgrading from a previous version, or installing for the first time, installation is easy!  A single file called MMR.prc needs to be installed.  Install this file as you would any other Palm application.  If you have not installed any Palm applications before, refer to the manual that came with your handheld.

 

Note:  If you are currently running a 1.0 Beta version, you must install and run once, version 1.0 release before installing version 1.1.  If you do not, you may lose some data.

 

About Auto-fill

My Medical Records offers an Auto-Fill feature for Doctors, Specialties, Clinics, Pharmacies, and Vaccines. Once you have entered an item in any those fields, it will popup automatically next time you go to write it again!

 

About Sorting

My Medical Records offers sorting on many of its lists!  The sorting can be accomplished by selecting the “Sort by…” option off the View menu associated with a list.  The first time you select a sort, the information in the list will be forward sorted (A-Z, or 0-9).  If you select the same sort again, the information in the list will be reverse sorted (Z-A, or 9-0).  The following views support sorting:

Starting My Medical Records

 

Just tap on MMR to start My Medical Records

 

 

 

Patient List

 

The first time you start My Medical Records, it creates and initializes some databases it needs.  This may take a second or two, and then you are presented with the Patients List screen.  It is here where you can create new Patients, or select existing Patients for viewing or editing.  You can sort this view by either the patient name or the patient’s birthday.

 

Tap the New button to create a new Patient.

 

Tap a Patient’s name to view that Patients medical history.

 

 

 

Patient Details

The Patient Details form is where you enter general information about each patient.  You may enter as little or as much information as you like.  I suggest entering at least the last 4 digits of the patient’s Social Security Id in the Medical Id field, as many medical providers require that information.  When you are done entering information, click the OK button and you will return to the Patient List

 

 

 

Patient Information – General

You are taken to this form after clicking on a Patient in the Patients List.  This form holds general information about the patient, and is a read only version of the information on the Patient Details form.

 

 

Clicking on General reveals the other forms.

 
                                              

 

Clicking on the Done button returns you to the Patient List form.

Clicking on the Details button takes you to the Patient Details form.

Clicking on the drop down menu (Information List) in the upper right hand of the screen presents you with a choice of what further information you wish to view or add.

 

Patient Doctors

Selecting Doctors off the Information List brings you to the Patient Doctors List.  This list shows the doctors the Patient has seen and their specialties.  Any name appearing in Bold means that Doctor has been marked as a Primary Care Physician.  You can sort this list by either the doctor’s name or the doctor’s specialty.

 

 

Selecting a Doctor on this list, or clicking on Add, takes you to the Doctor Details form.

Selecting another item from the drop down Category List will take you to other forms.

Clicking on the Done button takes you back to the Patient List.

 

Doctor Details

This form allows you to enter a Doctor’s name and specialty.  This form supports Auto-Fill, so if you have previously entered either a Doctor Name or Specialty that information will appear as you enter the information.  One of the databases My Medical Records created at start up holds many typical medical specialties, so you may never have to fully enter one yourself. Of course if you do enter one that did not previously exist, My Medical Records will remember it so you will not have to enter it again!

 

 

Immunization List

This form lists all Immunizations the selected Patient has received.   You can sort this list by either the immunization date or the vaccine name.

 

 

Click on an Immunization name to view the details, or select Add to add a new Immunization.

Pressing the Done button will take you back to the Patient List.

 

Immunization Details

This form shows the Name of the Vaccine, the Date it was given, and the Clinic where it was given.  You may add a note if you wish to record other information such as Lot number or Manufacturer.  Both the Vaccine Name and the Clinic fields support Auto-Fill so you do not have to enter information you have previously have.  Another of the databases created when My Medical Records was started for the first time is one that holds many common Vaccines, so you may never have to fully enter one yourself.  You may, of course, also enter a Vaccine not automatically listed by My Medical Records.

 

 

 

Prescription List

This list shows the Prescriptions that the Patient has been given.  A menu option allows you to view either all the Prescriptions, or only the ones marked as active.  If you are viewing all the Prescriptions, the ones marked active will appear in a bold font.  You can sort this list by either the prescription date or the medication name.

 

       

 

Selecting a Prescription on this list, or clicking on Add, takes you to the Prescription Details form.

Clicking on the Done button will return you to the Patient List.

 

Prescription Details

This screen shows the details of the selected Prescription.  You may enter as much or as little information as you wish.

 

 

 

Allergy List

This list shows Allergies the Patient has.  A menu option allows you to view either all the Allergies, or only the ones marked as active.  If you are viewing all the Allergies, the ones marked active will appear in a bold font.  You may sort this list by either the starting date or the allergy name.

 

  

 

Selecting an Allergy on this list, or clicking on Add, takes you to the Allergy Details form.

Pressing the Done button will return you to the Patient List.

 

Allergy Details

This form allows you to edit an Allergy.

 

 

Conditions List

This list shows any Medical Conditions the Patient has.  A menu option allows you to view either all the Conditions, or only the ones marked as active.  If you are viewing all the Conditions, the ones marked active will appear in a bold font.  You can sort this list by either the condition start date or by the condition name.

 

  

 

Selecting a Condition on this list, or clicking on Add, takes you to the Condition Details form.

Pressing the Done button will return you to the Patient List.

 

Condition Details

This form allows you to enter a name, date and description as well a note and say whether the condition is active or not.

 

 

 

Medical Event List

This list shows Medical the Patient has experienced.  You may sort this list by either the event date or the event name.

 

 

Selecting an Event on this list, or clicking on Add, takes you to the Medical Event Details form.

Pressing the Done button will return you to the Patient List.

 

Medical Event Details

This form allows you to enter an Event name and date.  You can also enter the name of any Doctor involved in the treatment as well as the Clinic name.  Both the Doctor and Clinic field support Auto-Fill.

 

 

 

Medical Exam List

This list shows Exams the Patient has had. You may sort this list by either the exam date or the doctor’s name.

 

Selecting a Prescription on this list, or clicking on Add, takes you to the Medical Exam Details form.

Pressing the Done button will return you to the Patient List.

 

Medical Exam Details

This form allows you to enter the date, Doctor, and Clinic for an Exam.  You can store more information such as results in the note field.  Both the Doctor and Clinic fields support Auto-Fill.

 

 

 

 

Insurance List

This list shows Insurance the Patient has.

 

 

Selecting an Insurance on this list, or clicking on Add, takes you to the Insurance Details form.

Pressing the Done button will return you to the Patient List.

 

Insurance Details

This form allows you to enter the type of insurance, the company name, subscriber Id, and an account number.  The insurance Types are: Dental, Disability - Long Term, Disability - Short Term, Life - Term, and Life - Whole.

 

 

 

Other Forms

There are four other forms that show the data used by the Auto-fill fields in forms.  From them, you can correct spelling, add new records, or delete records.  You do not need to ever use these forms to make the Auto-Fill work, but they are provided for your convenience.

Clinic List and Details

These forms allow you to add, delete, or modify Clinics.

 

  

 

Doctor List and Doctor Details

These forms allow maintenance of Doctor Auto-fill records.  You can add, delete or modify Doctor Records.

 

  

 

 

Pharmacy List and Pharmacy Details

These forms allow maintenance of Pharmacy Auto-fill records.  You can add, delete or modify Pharmacy Records.

 

  

 

Vaccine List and Vaccine Details

These forms allow maintenance of Doctor Auto-fill records.  You can add, delete or modify Doctor Records.

IN NO EVENT WILL THE AUTHOR or AUTHORS BE LIABLE TO YOU FOR ANY DAMAGES, INCLUDING INCIDENTAL OR CONSEQUENTIAL DAMAGES, ARISING OUT OF THE USE OF THE SOFTWARE, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
